PROGRESSION OF CARE COORD-ARNP - SAH - FT DAYS(Job Number: 168253)Description The Progression of Care Coordinator-ARNP is a Nurse Practitioner that coordinates Length of Stay initiatives for high risk patients throughout assigned hospitals. The Coordinator is responsible for receiving initial referrals to the program, conducting/coordinating an initial assessment, facilitating consults to medical support when necessary, and coordinating the work of the interdisciplinary team. The Coordinator also leads the program's strategic planning around team member and/or patient and family education initiatives to efficiently manage length of stay and reduce readmissions.
In addition, the Nurse Practitioner also functions as an active participating member of the interdisciplinary care team to advance progression of patient care toward discharge. This role will also address transition of care needs for high risk patients along with assisting in the development of the plan of care with the patient o maximize POC compliance, involving Home Care, DME, Pharmacy and communication with physicians.Qualifications Certifications and LicensuresRequiredARNP (Nurse Practitioner)
