Our client is a specialty pharmacy that provides high-touch services and specialty pharmaceuticals for patients.  Their organization is looking for a Nurse - RN - LPN with a minimum of 2 years of Specialty Pharmacy Experience to join their team!

 

 Nurse Job Summary:    
Responsible for the communication and caring for patients receiving specialty infusions as well as patient receiving specialty pharmacy therapies. 
 
Nurse - RN - LPN Responsibilities: 
Responsible for the coordination of care for all patients to receive Specialty infusion and Specialty Pharmacy Service from intake referrals to continued refill follow up and clinical management.

  • Obtain, enter and update referral information in EMR.
  • Discuss insurance limits and parameters of policy with patients prior to beginning services. 
  •  Nurse acts as a liaison between home health agencies, home infusion center and physicians to coordinate discharge, plan for patient care and establish Plans of Treatments 
  • Nurse will oversee the referral process and maintenance of EMR.
  • Coordinate therapeutic monitoring pertinent to therapy with home health care agency, laboratory and specialty infusion center pharmacy.
  • Coordinate pharmacy dispensing schedule with delivery of drugs and supplies.  Verify disposable supplies are packed and are appropriate for patient needs. 
  • Coordinate with home health care agency, laboratory and specialty pharmacy for injectable self administration teaching, lab coordination and shipment arrangements 
  • Coordinate patient care at refill establishing current response to therapy and ADR/Adherence occurrences, forwarding all clinical calls to appropriate clinician, under the supervision of the pharmacist.
  • Nurse is responsible for all activities and duties including but not limited to appropriate communications via progress notes, arranging nursing, patient communications, health care professional communications, completing scheduling board, as well as scheduling shipments via patient contact and communication in computer system, e-mail system and verbally.
  • Coordination involvement and auditing of requirements of specific clinical programs as per policy and procedures. (REMS, IgG, Hemophilia, etc.)
  • Participate in center QA&I to ensure achievement of excellence.
  •  Nurse is responsible for complying with all Policies and Procedures and Pharmacy and Nursing laws and regulations.
     


QUALIFICATIONS & ABILITIES:

  • Registered Nurse or Licensed Practical Nurse, currently licensed in state in which employed.  CRNI credential preferred.
  • At least three years nursing or Specialty Pharmacy experience. Prior IV therapy experience preferred.
  • Ability to work cooperatively.
  • Good verbal and written communication skills 
  • Good computer skills.

 
REQUIRED EDUCATION/TRAINING AND/OR PROFESSIONAL EXPERIENCE:

  • Registered Nurse or Licensed Practical Nurse, currently licensed in state in which employed. 
  •  At least three years nursing or Specialty Pharmacy experience.  Prior IV therapy experience preferred

 

EXPECTED PROFESSIONAL COMPETENCIES:
 Participate in any clinical program competencies as developed within company Policy and Procedures.


LICENSURE/CERTIFICATION REQUIREMENTS:
Registered Nurse or Licensed Practical Nurse, currently licensed in state in which employed. 
 
CONTINUING EDUCATION:

  • Meets any continuing education requirement of company clinical program as defined within company’s policy and procedures.
  • Stays abreast of developments in field of nursing, specialty infusion, injectable and oral  therapies

 
Meets State Board of Nursing, Health and Rehabilitative Services and accreditation agency’s requirements.
