Responsibilities:
 

The supervisor is primarily responsible for the management, supervision and coordination of the care coordination team. Provides leadership to the team and serves as a subject matter expert in care coordination, clinical program concepts, methods, and activities. Build successful relationships with Home Health providers, Prescribed Pediatric Extended Care facilities,treating physicians, hospital nurses, case managers, discharge planners, social workers and families.

More Information:

1. The successful candidate will have an active, unrestricted Florida registered nurse license with at least 2 years supervisory experience.
2. Five (5) years of clinical experience or combination of clinical experience and utilization review
3. Relevant pediatric experience required and some Medicaid background preferred
4. Bi-lingual Spanish/English preferred
5. Successful candidates will be able to work independently, be proficient with Microsoft Office products and have the ability to prioritize and meet timelines.
6. Interactions with families may require evening or weekend commitments. These are generally work out of the home positions.
