JOB SUMMARY: Responsible professional work delivering family support services utilizing a broad range of community services and natural support systems.  Involves a broad range of training and development activities, assessment services, and consultation and requires quality documentation of services provided.  Participates in and provides leadership in the organization’s continuous quality improvement program.

ESSENTIAL FUNCTIONS:

  1. Provides support to Project Director with program operations.  Responsible for data integrity and tracking and monitoring program outcomes. 
  2. Maintains a full caseload of 12-15 families.
  3. Maintains weekly contact with all families on active caseload by means of home visits, office visits, phone contact or correspondence.
  4. Facilitates support group for relative caretakers regularly.
  5. Assists families in developing and accomplishing Support Plan Goals.
  6. Supports children living with relative caregivers to remain outside of foster care.
  7. Creates, implements, and evaluates effectiveness of each family’s support plan.
  8. Completes assessments, program statistics and other required documents in the specified amount of time as per contract expectations.
  9. Ensures that families have follow-up linkages into the community once the family has completed program.
  10. Follows-up on all referrals to determine if family received services.
  11. Demonstrates leadership role as necessary or indicated (i.e. identifying challenges or issues within the program; participation on project and/or team/peer committees as directed; acts as a positive representative for the program)
  12. Serves as a mentor for other Family Support Coordinators.
  13. Serves in a leadership capacity through participation in committees, trainings, etc.  May conduct training as directed.
  14. Meets with Project Director on a weekly basis for supervision to review cases, training and professional development and program development.
  15. Works within the philosophy and mission of The Children’s Home, Inc.
  16. Participate in quality assurance reviews of the program services, outcomes and records.
  17. Other related duties as assigned.

MINIMUM QUALIFICATION REQUIREMENTS:

Education & Experience:       

Master’s degree from an accredited university or college in Human Services or related field and one (1) year experience working with children and families; or Bachelor’s Degree from an accredited university or college in Human Services or related field, with three (3) years of experience working with children and families.

Licenses & Certifications:      

 

  • Must possess and maintain a valid Florida driver’s license with no record of criminal driving offense of license suspension. 
  • Must possess means of providing job related transportation and show proof of required insurance.  Must be insurable under CHI’s current auto insurance policy.
  • Must be able to work flexible hours, including evenings and weekends. 
  • Must successfully complete and maintain FDLE clearance, Federal background and state criminal background check and sexual predator screening.

Knowledge, Skills and Abilities:         

  • Ability to communicate effectively, verbally and in writing. 
  • Ability to establish effective working relationships with family’s, coworkers, supervisors, and other individuals. 
  • Possess effective interpersonal skills. Knowledge of agency’s organizational structure, standard operating procedures, and policies.
  • Knowledge of child abuse or neglect reporting procedures and methods.
  • Knowledge of intervention and behavior management methods, strategies, and techniques.
  • Knowledge of quality documentation as required by agency standards, rules, and regulations.
  • Ability to conduct assessments to plan treatment services, and to provide follow-up services.
  • Ability to work as a multi-disciplinary team member in a positive productive manner.
  • Knowledge of agency's organizational structure, standard operating procedures, and policies.

ESSENTIAL PHYSICAL SKILLS:

Visual: ability to safely operate a motor vehicle, read printed materials, and to make observations of physical facilities.

Hearing: ability to understand and comprehend spoken dialogue in individual and group settings (with or without hearing device).

Ability to communicate clearly and effectively.

ENVIRONMENTAL CONDITIONS:

Work indoors in an office environment.

Works in the community and conducts home visits

High level of emotional discussions, requiring self-awareness and understanding of professional boundaries.

SUPERVISION RECEIVED:

Weekly individual and team supervision during which time training is provided, individual families concerns are addressed, and individual areas of professional growth are reviewed.
