The position of RN Case Manager works closely with the PCP to manage the MAPD/ ACO patient. Critical thinking skills and the ability to think outside the box are a necessary skill. Interaction with multiple agencies and departments within and outside the company on a professional level are expected. Patient education must be done keeping in mind the patient’s level of healthcare literacy. It is expected that the RN Nurse Case Manager will keep the immediate supervisor informed of absences by telephone, text, or email, and follow the appropriate chain of command. Position reports to Clinical Operations Manager
Nurse Case Manager Essential Duties and Responsibilities
- Meet with all new MAPD patients
- New Patient Template completion in CPS
- Advanced Directives discussion
- Review hospital admissions and discuss with UM
- Monitor patient progress/ anticipate discharge plan/ discuss w/ Inpatient CM as needed
- Verify information is correct in PCMS (Autoroutine)
- Transitions Follow up Phone Call and/ or Office Visit
- Hospital, SNF, post- procedure
- 24-48 hours after discharge
- documented in PCS TOC (Transitions of Care) template
- Face to Face Medication Reconciliation
- Review daily office schedules to determine patient follow up
- Social issues
- Advanced directives
- Disease education- possible Disease Management enrollment
- Hedis/ Quality Measures monitoring/ follow up
- Coordinate hospital/ SNF admissions as directed by PCP
- Elective procedures
- Review w / PCP and referral coordinator for appropriateness
- Pre-op teaching
- Discharge plan post procedure (SNF, home, HHC)
- Maintain weekly updates in PCMS of all SNF patients
- Address discharge plan
- Verify last covered day and discharge
- Maintain monthly PCMS list updates
- ESRD/ Hospice/ Oncology
- Notify weekend on call RN Case Manager of patients requiring outreach calls
- Review high cost/ frequent flyer patients w/ UM
- Plan of care to include regularly scheduled outreach calls
- Assist in triage of MAPD patients telephonically or as walk-ins
- IV therapy / phlebotomy/ dressings as per course
- Participate in Weekend Call Team on scheduled basis
