The Social Service Specialist I is a member of a collaborative practice model that includes patients, nurses, clinical resource coordinators, physicians, and other practitioners, caregivers, and the community. They are knowledgeable about disease, treatment, social and economic factors that impact individuals and families as it pertains to their age, stage of development and discharge needs. Their goals include achievement of optimal health, access to care and safe transition of care through the continuum balanced with the patient's right to self-determination.
