Description

The Manager, Care Partners is the lead clinical expert on the Continuous Care Management team, responsible for building a team of care coordinators who manage the care development plans and transition of care of patients across the healthcare continuum. The Manager routinely interacts with physicians participating in the ACO/CIN as well as their office staff, practice transformation team and Medical Directors.

The Manager, Care Partners develops and manages the Care Coordination function, including policies & procedures, managing the department’s budget/forecast, care coordination orientation program for the care partners, physician, office staff, as well as ongoing management of the care coordination services. The Manager leads, supervises, and directs the workload of the Care Partner team and will be responsible for the job evaluations / performance reviews for each Care Partner team member.

The Manager promotes BayCare’s Enterprise Care Management vision, helps sets the departments’ strategic directions and goals, oversees overall financial management, planning, systems and controls, collaborates with other departments and community leaders to achieve efficiency and effectiveness throughout the continuum. Supervises daily activities, conducts daily Care Team Rounds, and leads key strategic projects/program development.

2 years of Care Management experience is preferred. 2 years of Financial Management experience is preferred.
