Description

The Social Service Specialist II is a member of a collaborative practice model that includes patients, nurses, clinical resource coordinators, physicians, and other practitioners, caregivers, and the community. They are knowledgeable about disease, treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs. Their goals include achievement of optimal health, access to care and safe transition of care through the continuum balanced with the patient's right to self-determination.

This position will require travel to Winter Haven Hospital, Bartow Regional Medical Center and South Florida Baptist Hosital.
