The LPN works in collaboration with and under indirect coordination and oversight of the RN Care Partner Coordinators to assist with contacting patients and providers telephonically to improve the health, quality of health care services and experience of the populations served by the network. The LPN will be Responsible for reviewing daily Excel reports, outreaching to patients and providers and supporting the efforts of BayCare Physician Partners. Will perform other duties as assigned. Potential travel within the BayCare service area using own transportation.
