Description

Case Management is a collaborative practice model including patients, nurses, social workers, physicians, and other practitioners, caregivers, and the community. The Care Coordination process encompasses communication and facilitates care along a continuum through effective resource coordination. The goals of Care Coordination include the achievement of optimal health, access to care and appropriate utilization of resources balanced with the patient?s right to self-determination. Required experience includes 1 year of evidence-based clinical decision support or completion of Case Management Certification or completed Case Management internship, in addition to 3 years of Healthcare experience.
