FUNCTION:  
This member of the Assisted Outpatient Treatment (AOT) multidisciplinary team is responsible for conducting medical health and wellness assessments, assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing of a range of treatment, rehabilitation, and support services; and sharing shift-management responsibility with other staff.

KNOWLEDGE AND JOB SKILLS:
Must have a clear understanding of the characteristics and problems of adults with severe and persistent mental illnesses substance use disorders and be knowledgeable about the AOT model. Demonstrates excellent verbal and written communication skills. Displays excellent telephone and customer service skills.

JOB REQUIREMENTS:

Education: Completion of an accredited nursing program for Licensed Practical Nurses

Experience: Minimum of 1 year experience with the severe and persistent mental illness population or related field.

Personal Qualities: A pleasant and friendly manner is vital to satisfactory job performance. Must display patience and understanding of person's with disabilities and in challenging customer service situations. Must be alert and vigilant at all times. Dependability and adaptability essential for this position. Must show initiative and be able to work autonomously.

Supervisory Responsibilities: None.

Special Factors: Maintains a professional atmosphere. Provides excellent customer service. Maintains good working relationship with center, as well as, facility staff and external agencies. Works with staff psychiatrist and ARNP to coordinate pharmacological management.

Essential Functions: Nursing assessments and interventions. Documentation of services provided. Successful functioning in a team-based program.

Staff Development: Completes all administrative and competency requirements in accordance to Individual Training Plan.
