ESSENTIAL FUNCTIONS: Exercises a direct services role with the Hospital Recovery Team to ensure provision of high-quality clinical/case management services to facilitate recovery for LifeStream Behavioral Center Hospital consumers, including 1) treatment planning that addresses identified consumer needs; 2) face-to-face interventions to assess consumer progress and emergent issues; and 3) discharge planning that effectively links consumers to appropriate community services and resources.  

Assesses consumer needs, actively participates in treatment team planning, monitors consumers’ legal status, collaborates and implements consumer discharge plans, and other functions as explained in the Responsibilities portion of the job description at the discretion of the Lead Recovery Specialist.  

RESPONSIBILITIES:  

  1. Maintains a direct services caseload as assigned and provides a comprehensive range of clinical/case management services to maximize potential for consumer recovery.
  2. Assesses consumers on assigned caseload through face-to-face interventions on a daily basis to identify presenting and emergent issues and service needs.
  3. Develops treatment plans for consumers on caseload within twenty-four (24) hours of admission that address identified treatment issues as identified in emergency evaluations, psychiatric evaluations and face-to-face interventions with those consumers.
  4. Develops discharge plans on behalf of consumers on assigned caseload that address identified consumer needs and links consumers to community resources/services as appropriate and necessary.
  5. Documents direct services to consumers in the electronic medical record known as TIER timely, accurately and comprehensively.
  6. Participates in treatment team meetings as scheduled and provides salient information to the team on behalf of assigned consumers that communicates treatment progress, forecasts future course of treatment and facilitates discharge planning.
  7. Initiates and completes all required legal paperwork in compliance with Florida Statute 394, commonly referred to as the Baker Act.
  8. Ensures continuity of care for assigned consumers by scheduling inter-program case staffings (IPCS) to plan and coordinate with community outpatient service providers as necessary and appropriate.
  9. Conducts psycho-educational groups with consumers as assigned by the Lead Recovery Specialist to ensure Recovery Team group service provision as scheduled in the context of the Hospital Master Schedule.
  10. Attends all meetings as assigned by the Lead Recovery Specialist and/or the Director of Emergency & Social Services, or otherwise by LifeStream.
  11. Completes staff development training as assigned according to the established LifeStream schedule.

 

KNOWLEDGE AND JOB SKILLS: Possesses a working knowledge of psychiatric disorders and their treatment. Possesses knowledge of legal and ethical issues in the treatment of psychiatric patients, particularly with regard to all laws pertaining to the voluntary and involuntary treatment of individuals deemed to be in danger of harming themselves or others by their behavior, mental illness or drug and/or alcohol abuse as specified in the Baker and Marchman Acts. Utilizes knowledge of the array and scope of services offered within the Center and in the community. Possesses excellent interviewing and crisis intervention skills are essential to this position. Possesses good written and spoken communication skills, as well as excellent time management skills. Demonstrates sound computer skills, including a working knowledge of Microsoft Office.

JOB REQUIREMENTS:

  • Education: Master’s Degree preferred. Minimum of a Bachelor’s Degree in social work, counseling, psychology, or other behavioral healthcare/human services-related field required.  
  • Experience: Minimum of one (1) year in a behavioral healthcare/human services-related setting.  
  • Personal Qualities: Possesses the capacity for autonomy, self-direction, organization and detail-orientation. Possesses excellent interpersonal skills and the ability to collaborate effectively with individuals from multiple disciplines. 
  • Supervisory Responsibilities: None. 
  • Special Factors: Cooperates with co-workers and others in the community in such a manner that is conducive to the well-being of clients and insures proper maintenance of continuity of care and confidentiality. Generates concise, accurate and timely documentation.
  • Staff Development: Completes all administrative and competency requirements as outlined in the Individual Training Plan.
