Lifespace Communities would like to introduce the opportunity to become a Director of Home Health, at Village on the Green located in Longwood, Florida. Village on the Green is an upscale, gated, residential community located on an 80-acre site, which includes a 20-acre nature preserve. The community contains 42 villas, 202 one and two bedroom apartment homes and health center. The amenities includes excellent cuisine at our fine dining restaurant as well as housekeeping, maintenance, 24 on-site security, transportation and quality health care.  

The Director of Home Health is responsible for the planning, organizing, directing and marketing the home health department.   

A few other details about the role:

  • Ensure resident satisfaction levels are achieved through quality and hospitality initiatives.
  • Market home health services to residents.
  • Review resident referral information and respond to home care service requests and inquiries.
  • Promote home care and wellness programs by conducting resident clinics and sponsoring resident trainings.
  • Direct resident care, conduct resident assessments and develop plan of care, collaborate with physicians and other health care professionals in implementing and maintaining plan of care, assist with medication administration and treatments. 
  • Evaluate as needed the physical, emotional and social needs of residents.
  • Monitor resident for changes in condition, medication & nutritional needs and update care plan, physician and family as necessary.
  • Responsible for oversight of all agency team members including hiring, supervising, training, performance management and corrective action.
  • Review chart documentation and service agreements to ensure compliance with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all company policies and procedures.
  • Policy development and monitoring.
  • Team member engagement, relations, recognition and retention.
  • Budget preparation and forecasting including capital expenditures, cost control, inventory and variance reporting.
  • Coordinate with departments to ensure services are provided, maintain list of hospitalized residents to confirm that services continue upon discharge.
  • Provide leadership and vision by developing long range and annual plans meeting Lifespace and community strategic plans.
  • Implement, evaluate and report on progress of plans.
  • Ensure nursing services are provided according to the resident's plan of care with home health access available 24 hours per day, 7 days a week.
  • Develop and maintain an on call coverage system for nursing and resident care services that provides coverage on evenings, nights, weekends and holidays.
  • Participate in the home health advisory committee and development of administrative and resident care policies and procedures relating to the agency.
  • Direct implementation of approved work methods and procedures that reflect elements essential to rendering high quality care.
  • Participate in the review, analysis, and appraisal of the effectiveness of the total agency program.
  • Ensure the accuracy of public information materials and activities.
  • Ensure  appropriate  competency  assessments  and  evaluations  are  completed  on  agency  personnel as required by accreditation standards and state regulations.
  • Establish staffing patterns, which reflect the quality and quantity of various personnel necessary to plan, provide, and supervise the care rendered to residents and families.
  • Establish methods for coordination of care by all disciplines.
  • Ensure in home supervisory visits of non-licensed personnel and LPN in accordance with standards, regulations and/or policies.
  • Assist  agency  administrator  with  the  development  and  implementation  of  the  agency wide performance improvement program and assists the administrator with the annual evaluation of the total agency program
  • Review and evaluate the effectiveness of the agency infection control program and participates in the identification of risks including infections and communicable diseases
  • Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all company policies and procedures.
  • Obtain knowledge and demonstrate the principles of Living Lifespace , person directed care and successful aging philosophies and their inherent core values of commitment, respect, compassion, dedication, team work and quality. Team member will support, actively participate and act in accordance with the principles of the Living Lifespace service culture and be familiar with and fluent in the language and service pillars of Living Lifespace.
  • Maintain regular and punctual attendance at work and meetings.

And here’s what you need to apply: 

  • Licensed Registered Nurse in the applicable state.
  • Three years of experience as a nurse with two years administrative/supervisory experience in home health or community health.
  • Completion of a bachelor’s degree in nursing or related field, highly preferred.
  • Completion of a master's degree program in nursing, including public health and/or administrative studies preferred.
  • Public Health Nursing certification, if required.
  • Experience leading a team is preferred.
  • Possess a thorough knowledge of the federal, state and local laws, regulations, and guidelines for resident services, including HIPAA and resident rights.
  • Possess thorough knowledge of care planning, and documentation processes.
  • Proficient in computer use, including internet, word and excel.
  • Solid organizational skills including attention to detail and multi-tasking skills.
  • Strong written and verbal communication skills.
  • Ability to read, write and speak English sufficient to perform job duties and to interact and communicate with residents, vendors and team members.
