Essential Functions: Day-to-day supervision of licensed and non-licensed personnel. Participate as requested in the interviewing and hiring of nursing associates. Exercise independent judgment in evaluating the performance of nursing associates on unit. Maintain consistent practices and effectively communicates performance expectations. Assigns specific duties for resident s care, work hours, breaks, and meal periods for nursing associates on unit. Ensure proper staffing by calling in replacement employees for staff not reporting for work and/or by transferring or reassigning staff as necessary. Assure staff are making rounds every two hours or as required by resident plan of care. Verify showers are completed by CM/C.N.A. s daily and resident care is completed, ensures that the residents are assisted at meal/meals are served to resident room. Perform other management duties as assigned. Oversee the thorough evaluation/assessment of residents to determine their physical, emotional status and document in the Medical Record. Monitor resident activity, evaluate and direct nursing care in accordance with professional standards, and according to the physician s orders, care plans, established standards and facility policies. Coordinate and/or assist with preparation for new admissions, transfers, and discharge or residents. Review transcribed physicians orders for accuracy. Communicates with the physician, residents, family members, and other inter-disciplinary team members, to maintain current knowledge regarding resident status. Assist with scheduling first appointment with PCP, order medications, and transcribe medications onto MOR/TAR. Enter completed assessments in Yardi. Monitor medication administration as ordered by the physician; in accordance with nursing standards and facility policies. Perform chart and record keeping audits for completeness. Educate as needed on policies and procedures. Check all MORS daily for holes in charting, mistakes in transcribing orders. Maintain an updated knowledgebase in pharmacology. Demonstrate knowledge of policies governing medication administration and documentation. Demonstrate knowledge of drug reactions and sensitivities and nursing interventions. Assure that narcotics are secured and accounted properly. Educate individuals, family members or responsible party regarding the resident s medication regimen. Recognize normal and abnormal lab values and communicate to physician for direction, and other interdisciplinary team members as appropriate. Assist floor nurse with treatments, skin assessments and physician orders. Assists with ADL s, mobility and other functions as designated in the resident plan of service. Demonstrate working knowledge of infection control practices. Monitor, educate and evaluate Certified Nursing Assistants and other nursing associates on utilization of standard practices on infection control. Understand, comply with, train and promote compliance regarding Residents Rights. Promote positive relationships with resident, visitors, and regulators, to include presenting a professional appearance and attitude. Demonstrate safe practices in regard to resident s comfort and safety by applying knowledge or proper body alignment for self and resident. Practice and promote adherence to facility safety policy. Promote a restraint-free environment. Maintains expected ethical and legal standards including matters related to resident abuse or neglect, by personally complying with laws to safeguard residents. Reinforces through example and, where applicable provides training on the expectation that all employees that they supervise adhere to these ethical and legal standards, including related to resident abuse or neglect. When in doubt about whether a particular activity is proper, regains from the activity and obtains guidance from the Director of Nursing, or the Executive Director. Responds promptly and thoroughly when problems are reported by other employees. Assists in planning, implementing and monitoring corrective action, when problems including ethics and compliance are identified. Maintains compliance by promoting resident privacy, HIPPA regulations and confidentiality in the delivery of care. Safeguards patients medical financial and personal information. Assist and/or monitor the completion of the RAI and RAP summary and participate in developing and implementing the written care plan that addresses the needs of individual resident. Audit resident charts, making sure 1823 s are up to date, orders are in charts, all appropriate documents are in charts; bed rail forms, chemical restraints, PPD up to date. Participates in in-service training, assist in implementing orientation programs and job skills training; maintaining professional competence. Observes residents health status. Takes actions to address concerns within scope of practice and reports all significant changes, reactions to medications and treatment or significant incidents to DAL/attending physician immediately. Monitor environment and care practices of nursing associates to ensure compliance with established safety, fire and disaster, infection control and all other departmental policies and procedures are followed to ensure cleanliness and safety of work and treatment areas. Ensure equipment and work areas are clean, safe and orderly, and any hazardous conditions are addressed. Comply with, support and enforce all community policies related to safety and infection control procedures including the proper use of mechanical lifts, gait belts and personal protective equipment. Communicates to families regarding resident s personal needs and achievements. Reminds families of need to scheduled physician, dental or eye care appointments. Assist in organizing care plan review as required and assures reminders to family members are sent out. Ensures that in-house ancillary medical services such as podiatrist, doctor visits, etc. are scheduled and followed through Non-Essential Functions: Leads by example exhibiting the CORE Values through servant leadership Encourages teamwork and promotes company philosophy Attends required community meetings and participates Life Enrichment activities when able Completes all required courses in adherence with HRA University Is prompt and able to perform the required duties of the position on a regular, predictable basis Becomes familiar and understands how to report a missing person using the Safe Return program or equivalent Becomes familiar and understands the steps for fire evacuation All associates are responsible for maintaining a safe and secure environment for all community residents Qualifications/Skills/Educational Requirements: Current nursing (RN/LPN/LVN) licensure issued by appropriate state licensing agency Bachelor s Degree in health care related field Current s ECC and/or CORE Training certification as required Clinical experience, education or specialty skills specific to geriatrics is desired Must be able to relate positively and favorably to resident and families and to work cooperatively with others Related administrative experience at a level necessary to accomplish the job Must be capable of maintaining regular attendance. Present a professional appearance and behavior Must be able to communicate in a positive and professional manner Must meet all local health regulations and pass post-employment physical exam if required.
