Description

The Minimum Data Set (MDS) Coordinator is responsible for completing the MDS/Care plan and turnaround documents in a timely manner and communicate as needed with appropriate state departments. Conduct orientation/sessions on the MDS process to all team members involved in the process. Maintain documentation of all orientations/training sessions. Serves as a resource person for resident care plan activities. Work with accounts receivable personnel to facilitate payment. Participate in the Quality Improvement Process of the facility. Access continuing education opportunities related to responsibilities. Attend and participate in department meetings and perform other duties as assigned. Additional required skills include MDS, Care planning, Assessment skills.
