Description:
This position will serve as a Health Health Case Manager to maintain an active caseload of 15-20 clients with a minimum of once monthly face-to-face contact. Responsible for reviewing any existing medical reports/charts and consult with IMPOWER Physician or Physician Assistants as needed. This position will focus on both mental health and physical wellbeing. Will need to assess client/family strengths and needs then, in conjunction with the client, develop an individualized comprehensive “Healthy Options” care management plan to include diet and activity for each qualifying client.
Qualifications
- LPN or LCSW required.
- Successful background screenings through local law enforcement, Florida Dept. of Law Enforcement, and FBI.
- Successful motor vehicle background check.
- Valid driver’s license and auto insurance, insurable under IMPOWER auto policy.
- Bilingual Spanish required.
- 2 years of related experience required.
