Position overview:
Lutheran Haven Nursing Home is seeking to hire an MDS Coordinator for our operations in Oviedo, Florida. The ideal candidate should be an RN (Registered Nurse) with impeccable credentials as an MDS Coordinator.The position ensures that MDS assessments and care plans are accurately completed according to regulatory requirements and established deadlines.
Essential Duties & Responsibilities
- Coordinate and conduct the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment/evaluation.
- Meet with the nursing staff on all shifts, Interdisciplinary Care Plan Team, as well as support personnel, in planning the department's services, programs, and activities and gathering information for the resident assessment and care plan.
- Conduct or coordinate the interviewing of each resident for the resident’s assessment/evaluation.
- Evaluate each resident’s condition and pertinent medical data to determine any need for special activities or a need to amend the admission assessment/evaluation.
- Consult with the resident's family, and the resident's physician in planning the resident's care, treatment, rehabilitation, etc., as necessary.
- Review nurses' notes to ensure that they are informative and descriptive of the nursing care being provided.
- Schedule resident assessment/evaluation/care plan meetings.
- Develop preliminary and comprehensive evaluations of the nursing needs of each resident.
- Coordinate the development of a written plan of care for each resident.
- Assist the nursing staff in encouraging the resident and his/her family to participate in the development and review of the resident’s plan of care.
- Participates in periodically updating written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
- Develop, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans.
- Participate in facility surveys.
- Maintain the confidentiality of all resident care information in accordance with HIPAA policies and procedures.
- Provide information to the Quality Assurance and Assessment Committee as requested.
- Participate in regularly scheduled Medicare meetings. Implement recommendations from established committees as instructed by the Administrator.
- Pull the Quality Indicator Reports on a monthly basis and distribute a copy to Interdisciplinary Team.
