Position Overview: 

Make a real difference in coordinating access to services for children & families in Central Florida. Implement all aspects of the Targeted Case Management program in order to optimize the functioning of individuals who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner.  Complete intake process of all referrals in a timely manner.  Ensure assessments are completed within the appropriate timeframe and in accordance with criteria in policy and procedure, Medicaid handbook and FS Ch 65-E-15.  Develop service plans in accordance with criteria in policy and procedure, Medicaid handbook and FS Ch 65-E-15.  Work with the client and the client’s family to address issues related to the implementation of the service plan. 

Qualifications: 

  • Bachelor’s Degree in social work or a human services related field from an accredited college or university.
  • One year of full time or equivalent experience working with adults experiencing serious mental illness/children with serious emotional disturbances.
  • Has completed or agrees to complete AHCA-approved mental health targeted case management training within three months of initially providing Medicaid services.
  • Valid driver’s license and auto insurance.
  • Successful motor vehicle background check. 
  • Successful background screenings through local law enforcement, Florida Dept. of Law Enforcement, and FBI.
