JOB SUMMARY:

Responsible work in delivering family support services utilizing a broad range of community services and natural support systems.  Involves a broad range of training and development activities, assessment services, and consultation and requires quality documentation of services provided.

ESSENTIAL FUNCTIONS:

Family Support Coordinator will maintain a caseload of at least 12 families. Family Support Coordinator will maintain weekly contact with all families on active caseload by means of home visits, office visits, phone contact or correspondence. Family Support Coordinator will facilitate support group for relative caretakers regularly. Will coordinate with community partners to provide information and training to caregivers.

Families will accomplish 75% of Support Plan Goals who complete the program. 90% of children living with relative caregivers will remain outside of foster care one-year post discharge from the program Creates, implements, and evaluates effectiveness of each family’s support plan. Completes Assessments, program statistics and other required documents in the specified amount of time. Ensures that 90% of families have follow-up linkages into the community once completed program.

Family Support Coordinator follows-up on all referral to determine if family received services. Assists families navigate community resources and addressing barriers, including transportation, advocacy, legal, medical and mental health needs Will meet with Program Supervisor on a weekly basis for supervision to review cases, training and professional development and program development Participate in quality assurance reviews of the program services, outcomes and records

QUALIFICATIONS:

Education &Experience

Bachelor’s Degree from an accredited university or college in Human Services field, with one year of experience working with children and families.

Certifications:

Valid Florida driver's license and be insurable by our current insurance carrier.

Knowledge, Skills and Abilities:

  • Ability to communicate effectively, verbally and in writing.
  • Ability to establish effective working relationships with family’s, coworkers, supervisors, and other individuals.
  • Possess effective interpersonal skills.
  • Knowledge of agency’s organizational structure, standard operating procedures, and policies.
  • Knowledge of child abuse or neglect reporting procedures and methods. Knowledge of intervention and behavior management methods, strategies, and techniques.
  •  Knowledge of quality documentation as required by agency standards, rules, and regulations.
  • Ability to conduct assessments to plan treatment services, and to provide follow-up services.
  • Ability to work as a multi-disciplinary team member in a positive productive manner.

ESSENTIAL PHYSICAL SKILLS:

Visual: ability to safely operate a motor vehicle, read printed materials, and to make observations of physical facilities.

Hearing: ability to understand and comprehend spoken dialogue in individual and group settings (with or without hearing device).

Ability to communicate clearly and effectively.

ENVIRONMENTAL CONDITIONS:

  • Work indoors in an office environment.
  • Works in the community and performs home visits of client population to be served.
