healthcare provider, and social center designed for older adults with complex medical

Job Title: Activity and Day Center Coordinator (Florida PACE Centers)

Job Summary
Under the supervision of Pace Site Administrator, has full responsibility for all daycare
and activities throughout the adult day center; The Activity & Day Center Coordinator
manages the daily planning and coordination of center-based activities to include conflict
resolution, participant/caregiver care issues, certified nursing assistance and therapeutic
program.

Essential Job Functions
• Conducts initial assessment and scheduled reassessment of participants
• Develops specific therapeutic programs to include recreational and social
activities
• Records, signs, and dates progress reports in participant records
• Maintains the daily operation of the day center
• Plans and provides both group and individual programming
• Participates in interdisciplinary meetings and developing individualized and
measurable care plans
• Supervises with participant care needs such as toileting and feeding
• Supervises day care certified nursing assistants, volunteers, and additional
daycare staff
• Responsible for scheduling staff, toileting schedule and participant tracking record
• Keeps inventory and order sheets for recreation and day center supplies
• Plans and administers participant council
• Completes other assignments as needed

Job Requirements

Education:
• Bachelor's Degree in Therapeutic Recreation preferred; experience can be substituted for degree as indicated below.

Experience:
• 1-2 years' experience in recreational therapy with elderly population or Bachelor's Degree in Therapeutic Recreation and 1 year experience with elderly population.

Licenses/Certifications:
• Certified Activities Director preferred
• Certified Therapeutic Recreational Therapist preferred
• Certified Nursing Assistant preferred

Abilities Required
• Must be bilingual (English/Spanish)
• Friendly, outgoing, positive personality
• Excellent communication, organizational and interpersonal skills necessary to interact with all levels of the work group, and participants
• Sensitivity to the needs of an aging population
• Computer knowledge
• Knowledge of documentation desirable

Functional Demands

Environment Work Conditions:
• Normal working condition with adequate lighting and ventilation

Infectious Material Exposure:
• Occasional exposure to blood/body fluids/tissues
