PositionSummary:
TheCareManagerSupervisorisresponsibleforthedirectsupervisionandcoordination ofqualityservices andstaffofacase management teamfocusedontheprovision ofcasemanagement servicesforfamilies served.The CareManager Supervisorwill leadthe dependency casemanagement function with asavvyteam of5 DCMswho identify,assessandcoordinateservicedeliverysothatchildren/client needs,programgoalsandcontractobjectivesare met.TheCareManagerSupervisoradherestotheFPOCFSystemofCareprinciplesofpracticeandprovidesfamilycentered strength-based assessment,planning, linkage,coordination,and advocacyforchildren andfamilies.TheCareManager Supervisorworksasateam member withkeypartnersincludingparents,familymembers,foster parents,caregivers, guardian ad litem, DCF Protective Investigations, CLSattorneys, therapists, providers, and other appropriate professionals.
Technical/FunctionalExpectations:
1.Completerequiredcertification-Maintain own andstaff's ChildProtection Certification through completionof requiredin-servicetraininghoursandappropriatesupervisionactivities.
2.Safety-Conduct,Develop,Monitorand Managefamilyfunctioningassessments,childsafety assessments and required safetyplans.
3.Permanency-Identifyanddevelopplantosuccessfullyachievepermanencyforchildrenplacedinout-of-homecare.
4.ServiceDelivery-Assesschildandfamilyneeds; develop and monitor serviceplans;coordinate the deliveryof services;ensureallreferralsandlinkagestochildren/parents/caregiversarecompleted.
5.Reports -Monitor/Prepareallrequired courtreports and submitwithintherequiredtimeframes; make recommendations;testifyatcourthearingsregardingreunification,permanent guardianship,terminationof parental rights,or othercase management activities; ensure court orders are completed within required timeframes.
6.Documentation-Develop and maintain, case and program documentation according to contract; ensure documentation occurswithintheSACWISsystem andother relatedsystemswithinrequiredtimeframes. Oversee professional, timelyprogramcasedocumentation and reporting, includingthe trackingand monitoringofclient progressandoutcomes.
7.Engagement-Adherenceto thelocalSOCvalues,goalsandprioritiesfor Brevard'schildren,youth andfamilies; Utilizeafamilycentered strength basedapproach tocare;Monitorqualityofcarebyensuringface-to-face home visitswithchildrenarecompleted minimallyevery30days,orwithincreasedfrequency asdeterminedbycase situationsandsupervisorydirection;facilitate,monitor,andassessvisitationbetweenchildren,parents,andsiblings toensuresafetyandobserveinteractions.
8.Support-Providesupport tostaff,children,caregiversandfamilymemberstoassurestableplacement andtimely achievementofpermanency;Coordinatecaseswithteammembers,providers,DCFandattorneysasneeded.
9.CourtAppearanceandPreparation-Ensurestaffhavereviewedcaseinformation priortoeveryhearinginorderto bepreparedtoansweranyquestionsfromtheCourtregardingthestatusofthecase,services, andprogresstowards permanency;DressappropriatelyforCourtappearances andensureallcourthearingsareattended timelybyselfor assignedstaff.
