Responsibilities:
- Analyze high-cost medication orders and identify opportunities for cost savings through therapeutic interchange while ensuring patient safety
- Conduct comprehensive medication reviews to assess appropriate drug selection, dosage, administration, potential drug interactions, and opportunities for cost-effective therapy.
- Recommend appropriate therapy changes and therapeutic interchange options to physicians and facility staff.
- Evaluate medication orders for appropriateness of drug, dosage, potential interactions and proper administration
- Communicate effectively via, phone, and email.
- Utilize Microsoft Excel to run hourly reports.
- Complete any other task assigned by the Cost Containment supervisor.
Qualifications:
- Licensed Pharmacist in the State of Florida
- State license are plus PA, AL,OH, GA,NY
- Long Term Care or Hospital experience preferred
- Strong clinical skills, critical thinking, and analytical skills.
- Excellent written and verbal communication skills
- Strong attention to detail and organizational skill
- Framework / ECM software knowledge a plus
- Microsoft Excel knowledge skills
Shift:
- 8:00am - 4:30pm with rotating weekends and holidays.
- Onsite
