WHAT YOUR DAY ENTAILS
As our Population Health Manager, you will proactively work to improve the health status of populations living with chronic conditions by implementing and promoting wellness initiatives and evidence-based interventions. You will develop and maintain population health policies, collaborate with providers and community partners, and educate teams on best practices aligned with NCQA and Medicare standards. You will oversee vendor relationships, monitor compliance, and ensure programs meet regulatory requirements while supporting disease management initiatives and quality improvement efforts. You will analyze data quarterly to identify gaps, implement enhancements, and report outcomes. You will also oversee the Health Information Line, update educational materials, and participate in wellness assessments and community outreach activities to support healthier lifestyles.

QUALIFICATIONS TO BECOME OUR POPULATION HEALTH MANAGER
- Associate degree in Nursing (AA/AS) or equivalent from a two-year college, or one-year related experience and/or training, or equivalent combination of education and experience
- Registered Nurse according to Florida Statute 464
- 5+ years of related experience
- Current CPR certification
- Knowledge of HIPAA and other regulations
- Computer skills
- Ability to author, read, analyze, and interpret data and information
- Ability to work with mathematical concepts
- Ability to interpret an extensive variety of technical instructions
- Skill in caring for chronically ill patients
- Public speaking skills and experience
- Teaching skills
- Ability to work evenings and weekends, as needed and to travel locally
- Ability to communicate effectively and respond to information/questions
- Physical ability to occasionally lift and/or move up to 200 pounds (pushing a patient in a wheelchair)
- Experience with electronic medical records is a plus
- Bachelor of Science in Nursing (BSN) is preferred
