Job Summary and Qualifications
The Social Worker in the Senior Healthcare Center assists patients and families along the care continuum with social and emotional issues that may affect their medical care and well-being. The social Worker is responsible for case management, healthcare system navigation, as well as education and coordination of available and relevant community resources and referrals. Serves as a CMS-designated qualified health care provider completing services, documentation, and insurance-based requirements. Works with patients, families, physicians, staff, inter-hospital disciplinary team, and external community partners to facilitate plan of care.
This position is located at the Senior Healthcare Center Melrose office. Senior Healthcare Centers are outpatient departments of HCA Florida North Florida Hospital. Collaboration with other Senior Healthcare Centers and hospital department team members is anticipated. Travel to other company or community locations for meetings or events may occasionally be required.

Our Social Worker responsibilities:
• Assess, identify and coordinate plans of care and available resources to assist patient and caregivers
• Advocate for patient care choices with circle of care providers
• Coordinate referrals to community vendors, resources, and education
• Perform advanced care planning (advanced directive) discussions and education
• Counsel and support within scope of practice, with regards to social/emotional needs of patients/families/caregiver
• Perform MMSE's, MOCA, GDS, clock tests and other diagnostic screenings as ordered
• Initiate and lead family and patient conferences as appropriate
• Work in conjunction with community resources to further develop resources/programs to meet the social/emotional needs of community elders
• Maintain accurate, up-to-date documentation in the electronic medical record and departmental records
• Identify, create, and presents in-services and training for staff
• Participate in community outreach and education programs.
