Wellness Support Coordinator

PublishedPublished: 10/18/2026
Help turn a school into a place where students and families can find the support they need to thrive.

At Children's Home Society of Florida (CHS), we believe every child deserves the chance to dream big and to feel safe, supported and surrounded by opportunity, no matter their circumstances.

As a Wellness Support Coordinator, you'll help bring that vision to life within a Community Partnership School by connecting students and families with health, behavioral health and community resources.

This is more than coordinating referrals. You'll help build the connections between schools, families, healthcare providers and community partners that make it easier for students to get the support they need.

If you're organized, collaborative and passionate about helping children and families navigate services, this could be the opportunity for you.

What You'll Do

As the Wellness Support Coordinator, you'll coordinate and track the school's health and wellness referral network while serving as an important connection point for students, families and community partners.

You will:

  • Serve as a key point of contact between school administration, school nursing, medical clinics, behavioral health providers and other health-related partners.
  • Welcome students and parents into the Community Partnership School's network of support and help connect them with appropriate resources.
  • Coordinate and track referrals for medical, dental, vision and mental health services.
  • Follow up on referrals to help ensure students and families remain connected to needed resources.
  • Build referral relationships and linkages with community agencies and service providers.
  • Participate in Intervention Team meetings and support behavioral interventions and referrals.
  • Maintain timely and accurate assessments, treatment plans, progress notes, data and referral tracking.
  • Support crisis intervention when needed.
  • Help coordinate Health & Wellness events for students, staff and the surrounding community.
  • Support eligibility, billing and reimbursement processes.
  • Participate in quality-improvement efforts.
  • Coordinate or provide transportation for clients when needed.

Every referral you coordinate, partnership you build and family you connect with resources helps create a school community where children have greater access to opportunity.

That reflects CHS's broader commitment to comprehensive systems of support and nurturing environments where children can reach their potential.
